An opportunity has become available for a Clinical Nurse (Community Frailty Service) to join Gold Coast Health (GCH) where the focus of our strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia.
You will
Deliver comprehensive clinical assessment across a range of settings
Apply advanced clinical assessment skills, care planning and delivery of clinical interventions
Build and maintain effective working relationships
Work collaboratively with the RASS team members to enhance service delivery
Engage stakeholders to promote service awareness and use of RACF Clinical Care Pathways
About you
To be successful in this position you will require:
Bachelor of Nursing or equivalent
Registration with the Nursing Midwifery Board of Australia
Drivers Licence
Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
Tuberculosis screening, to identify if the tuberculosis vaccination may be required
Declaration of serious discipline history must be disclosed
Criminal History Screening will be required for new employees (GCH will cover cost)

About Us
This position sits with the Residential Aged Care Acute Support Service (RASS). The service operates across Gold Coast Hospital and Health Service sites and externally at Residential Aged Care Facilities (RACFs). Working in partnership with internal and external stakeholders, the service provides an emergency department (ED) substitution service where appropriate and in keeping with stated goals of care.
The service aims to improve the capacity of clinical staff and optimise the quality of care to residents of aged care facilities and improve the choice of care setting for residents with acute health care needs, where these needs exceed the scope of the General Practitioner (GP) and the RACF to manage independently.
RASS forms part of the Community Frailty Service, along with the Geriatric Emergency Department Intervention (GEDI), Advance Care Planning service and Community Dementia Service. These community focused teams work collaboratively to reduce avoidable ED presentations and hospital admissions for frail older adults.
