About the Role We are seeking a dedicated Care Coordinator to join our multi-disciplinary Health Care Services Team in Surry Hills. This telephonic role plays a vital part in supporting members who require hospital-substitute treatment and/or chronic disease management.
As a Care Coordinator, you will be responsible for facilitating timely access to appropriate care, and for coordinating the delivery of care, education, and support in collaboration with our broader care team.
What are we looking for? Tertiary qualification (preference in nursing but open to allied health). Current Registration with relevant professional association. Minimum 2 years (full-time equivalent) work experience utilising clinical qualification. A solid understanding of the principles that underpin coordinated care delivery. Knowledge of primary and other community care environments. Previous experience in chronic disease management. Health coaching is beneficial. Outstanding communication skills, verbal and written.
