The team will review each patient’s feasibility to convert from IVAB to POAB to facilitate earlier discharge, including liaising with HITH services to step patients down from treatment in the acute care setting.People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.Candidates Will Need To Meet The Following CriteriaCurrent registration as a pharmacist with the Pharmacy Board of Australia (or eligible for immediate registration) with no conditions or restrictions on practice and evidence of extensive and relevant hospital experience.Extensive experience in a hospital pharmacy practice including prior practice working in an antimicrobial stewardship environment.Demonstrated ability to initiate, lead, complete and share quality improvement activities or service evaluation process.Provide evidence of proficient written, verbal and interpersonal communication skills with the proven ability to liaise with novice and experienced health professionals and external service providers. Demonstrated computer literacy and ability to use health information systems.Demonstrated ability to initiate, lead, complete and share quality improvement activities or service evaluation process. Experience in clinical teaching and supervision of students, interns, Pharmacist, and/or Pharmacy Technicians/Assistants.Demonstrated proficiency in ability to work autonomously with limited supervision.
