The Role

* Creating Connection: Offering companionship, engaging conversation, and emotional support, while assisting with personal care needs in the comfort of the client's home.
* Helping with Travel: Assisting clients with transportation to important appointments, making sure they stay connected and supported in their healthcare journey.
Key Responsibilities
1. To deliver high-quality, person-centered support services to older adult clients in their own homes.
2. To work collaboratively with other professionals to develop and implement individualized service plans that meet the unique needs of each client.
3. To provide emotional support, companionship, and assistance with daily living tasks, promoting independence and well-being.
