Implementation and Learning Lead, Social Prescribing

About the role

We are seeking an Implementation and Learning Lead to support the establishment, growth and scaling of social prescribing initiatives across multiple projects and communities.

This role sits at the intersection of implementation, service development, evaluation support, project delivery and knowledge translation. It is ideal for someone who can work with community organisations, health services, referrers, researchers and project teams to understand what is working, what is getting in the way, and what practical supports are needed to help social prescribing become part of everyday practice.

The role will help sites establish and adapt local models, capture implementation learnings, support continuous improvement, and translate emerging insights into tools, resources, reports, training materials and scalable practice guidance.

Key responsibilities

The Implementation and Learning Lead will:

* support the establishment and implementation of social prescribing initiatives across multiple project sites
* work with local partners to understand referral pathways, workforce needs, community assets and implementation barriers
* capture practical learnings about what works, for whom, in what contexts, and why
* identify barriers and enablers to implementation, including issues with referral processes, forms, workflows, workforce confidence and local service capacity
* support sites to adapt and improve their models while maintaining fidelity to core social prescribing principles
* develop practical resources, including guides, templates, training materials, implementation checklists, scripts and project tools
* contribute to project reporting, funder updates, implementation summaries and learning reports
* work with evaluation and research partners to ensure implementation insights are captured alongside outcome data
* support workshops, interviews, co-design sessions and reflective learning activities
* synthesise information from meetings, site visits, interviews, workshops and project data into clear and useful insights
* help build a growing library of social prescribing implementation resources and practice materials

About you

You will be someone who can move comfortably between strategy and practice. You can listen carefully, make sense of complex information, and turn messy project learning into clear, practical tools that help people do the work better.

You may have a background in health, community services, public health, implementation science, social work, psychology, evaluation, project management, service design, community development or applied research.

You do not need to be an academic researcher, but you do need to be curious, organised, practical and able to write well.

Skills and experience

We are looking for someone with experience in some or all of the following:

* implementation, service development, project delivery or practice improvement
* health, mental health, community services, social prescribing, prevention, primary care or place-based initiatives
* supporting multi-site projects or community-based implementation
* developing practical resources, training materials, guides or toolkits
* capturing and synthesising qualitative learning from meetings, workshops, interviews or site engagement
* writing clear reports, summaries, briefing papers or project materials
* working with diverse stakeholders, including community organisations, health providers, funders, researchers and service users
* supporting co-design, consultation or reflective learning processes
* understanding barriers to referral pathways, workforce change, service uptake or local adaptation

Essential Criteria

* Relevant tertiary qualifications in health, public health, community services, psychology, social work, implementation science, project management or a related discipline, or equivalent demonstrated experience.
* Experience supporting implementation, service development, project coordination, evaluation support or practice improvement within health, community services or a related sector.
* Demonstrated ability to gather, synthesise and communicate practical learnings from projects, stakeholder engagement, workshops, interviews or service delivery activities.
* Strong written communication skills, including experience preparing reports, summaries, resources, guides, training materials or project documentation.
* Demonstrated ability to work collaboratively with diverse stakeholders across health, community, research, government or not-for-profit settings.
* Strong organisational and project management skills, including the ability to manage competing priorities across multiple projects and workstreams.
* Ability to identify implementation barriers, enablers and practical solutions within real-world service delivery environments

The successful applicant will be practical, collaborative and adaptable, with the ability to work across multiple projects, engage effectively with diverse stakeholders and translate complex information into practical, usable solutions. Strong listening, communication and relationship-building skills, together with a commitment to continuous learning, improvement and community-centred practice, will be essential to success in this role.

Experience or knowledge in areas such as social prescribing, implementation science, community development, primary care, PHN-funded services, mental health, children and families, chronic disease, co-design, qualitative research, evaluation, workforce development, service design or development of practical sector resources would be highly regarded.
