The Role
As a Support Coordinator, you'll be the vital link between our participants and the services that support them. You'll work closely with individuals, families, and service providers to bring NDIS plans to life — ensuring participants feel heard, supported, and empowered every step of the way.
What You'll Be Doing
· Coordinating and implementing participants' NDIS plans with clarity and care
· Helping participants connect with a variety of community, clinical, and mainstream services
· Building strong, respectful relationships with participants and their families
· Troubleshooting, problem-solving, and being a trusted advocate
· Monitoring progress and adjusting support as participants' needs evolve
· Writing reports, documenting outcomes, and liaising with the NDIA when needed

What We're Looking For
· Experience in the disability or community services sector
· Solid understanding of the NDIS and Support Coordination (Level 2 or 3)
· Exceptional communication and interpersonal skills
· Organised, proactive, and compassionate
· Relevant qualification in Social Work, Community Services, Allied Health, Psychology, or similar
· A current driver's licence, Working with Children check and NDIS Worker Screening Check (or willingness to obtain)
