About the Role
We are seeking a part-time Occupational Therapist (0.6 FTE / 22.8 hours per week), on a fixed-term basis until May 2026. This position is within the Aged & Community Care (ACC) Program, primarily based in Pakenham, with travel to various Monash Health sites as required.
You will work collaboratively within a multidisciplinary team to support clients in maintaining independence and living at home and within the community. Your responsibilities include conducting evidence-informed assessments, interventions, and discharge planning for clients in the Aged and Community Care Program. You will work directly with clients and/or carers to promote engagement in meaningful occupations and support healthy living, with services delivered in clients' homes, in the community, or via telehealth.
Your key responsibilities include:
1. Using specialist skills to conduct assessments and interventions independently, applying remedial, compensatory, educational, and preventative strategies.
2. Ensuring operational effectiveness while providing client-centered, compassionate care.
3. Building strong partnerships within and outside Monash Health, collaborating with Senior Clinicians and Team Leaders.

Our goal is to maintain clients’ independence, promote health and self-management, and intervene proactively to prevent deterioration, hospitalization, or residential care admission.
