About the Role

As a Support Coordinator, you'll walk alongside individuals as they navigate the NDIS and utilise their NDIS plans. You'll help coordinate supports, build capacity, and foster independence—ensuring each person has the tools and connections they need to thrive.

The position is permanent Part Time 60 hours a fortnight, SCHADS award Level 4.1

Key Responsibilities

* Build strong relationships with participants and their support network
* Build capacity of participants and their families to utilise and manage their NDIS plan
* Coordinate services and supports across multiple providers
* Monitor progress and NDIS funding allocations
* Advocate for participants and promote choice and control
* Maintain accurate documentation and reporting

What We're Looking For

* Experience in disability, mental health, or community services
* Relevant qualifications in Social Work, Community Services, Human Services or similar
* Strong understanding of the NDIS legislation, practice standards and guidelines
* Excellent communication and interpersonal skills
* Ability to work independently and manage time effectively
* Current driver's license and reliable vehicle (if applicable)
* NDIS Worker Screening Check and Working with Children Check

The successful applicant will have or be willing and able to obtain:

* Working with Children Check (Blue Card) or willingness to obtain
* Current NDIS Worker Screening check or willingness to obtain
* Demonstrated reliability, flexibility and a positive fun attitude
* Excellent communication skills including verbal, written and listening
* Competent using technology - Smart Phone and Apps
