About the Role
We are seeking a highly skilled and compassionate individual to fill the position of Assessor – Clinical Aged Care in our Caboolture region. This role involves conducting comprehensive assessments using the Integrated Assessment Tool (IAT) to evaluate and determine the needs of our customers seeking home support services.
The ideal candidate will have a tertiary qualification in a health-related discipline, current unrestricted AHPRA registration, and a minimum of one year demonstrated experience delivering services in aged care settings. They will also possess excellent customer service skills, adaptability, and flexibility to respond to changing task requirements.
Responsibilities include collaborating with customers, carers, families, representatives, and other service providers, undertaking and reviewing personalised support plans, identifying customers' formal and/or informal respite support arrangements, developing an Emergency Care Plan if necessary, referring for services, assessing complexity and risk of vulnerability, and assisting with linking support, reablement, and short-term case management.
This is a permanent, full-time opportunity with travel required in meeting with customers and stakeholders. Flexibility for part-time arrangements may be available with either a minimum 3 days per week or reduced daily hours across 4 or 5 days.

Key Qualifications
* Tertiary qualification in a health-related discipline (e.g., Registered Nurse, Physiotherapist, or Social Worker)
* Current unrestricted AHPRA registration or part of, or eligible to be part of, a relevant professional association
* Minimum of one year demonstrated experience directly delivering services in aged care settings and/or to aged persons
* Excellent customer service skills and ability to form positive relationships with community members and stakeholders
* Knowledge or experience in conducting home assessments or risk assessments for vulnerable people, including sensitivity for customers from diverse backgrounds and/or mental health conditions
