The RoleWe have an exciting and fast-paced opportunity to join the Infinite Clinical Care team as a Clinical Care Coordinator & Nurse Mentor. This full-time role reports to the General Manager, with support and guidance from the Clinical Care Manager, while supervising and supporting our passionate care team. The key purpose of this role is to ensure that all residents within our beautiful Care Home in Cornubia receive the highest quality of clinical care. The Clinical Care Coordinator is responsible for managing and resourcing the care needs within the facility, ensuring documentation and assessments are up to date and aligned with best-in-class clinical practices.Key ResponsibilitiesProvide clinical expertise and leadership to the clinical and care service teams across the Care Home.Assist the Clinical Care Manager in leading care teams to meet all clinical needs of residents, including monitoring and tracking clinical indicators to achieve optimal clinical outcomes.Manage assessment, documentation, and submission of AN-ACC requirements within the facility.Support effective documentation and ensure full clinical team support for policies, procedures, and IT system requirements.Coordinate assessment, care planning, and review processes within the facility.Support the admission of new residents, including assessments and initial consultations with residents and families.Ensure accurate recording of information in compliance with all organizational and legislative requirements (AN-ACC, resident records, staff time recording).Contribute to facility team rosters, ensuring an optimal skill mix for resident care.Manage and document clinical risks within the facility and promote continuous improvement.You will offerInnate empathy for residents and a commitment to improving clinical care quality.Current unrestricted Registered Nurse Div 1 registration with AHPRA.Postgraduate qualifications in aged care are advantageous but not essential.Experience and extensive knowledge of aged care clinical assessments, care plans, AN-ACC, accreditation compliance, and documentation.An engaging style capable of influencing and developing clinical leadership within the care team.High-level computing skills to maintain documentation and reporting in accordance with organizational and legislative standards.A confident team player with excellent communication and interpersonal skills, passionate about achieving positive clinical outcomes.Ability to manage multiple stakeholders with a balanced and level-headed approach to emergencies and stressful situations.Valid NDIS Workers Screening Clearance or the ability to apply for one via How to apply NDIS .Benefits - Why work for us?Fitness PassportOpportunities for career growth within a supportive mentoring cultureAccess to in-house and online training programs-free for all employeesSupportive, positive team environment with a strong caring work ethicCompanywide safety-first workplaces for residents, families, and staffPositive wellbeing activities, fun and engaging team/shift activitiesFamily-friendly benefits such as EAP and Flare (employee discounts)Work-Rite Micro professional development programs-free for all employeesWhy Infinite Aged Care?We are an Australian residential aged care provider with over 30 years of experience delivering quality aged care to the elderly.
