About The Role
This Enrolled Nurse role will be working within a multidisciplinary Transitional Aged Care Program (TACP) team, providing case management under the supervision of Registered Nurse.
The Aged Care Division delivers a range of clinical and non-clinical services across the Western NSW Local Health District to support older people and their families to remain supported in their home.
The Aged Care teams and programs have a strong commitment to advocacy, restorative care and reablement back to wellness and independence. The position will enjoy cooperation and support within a robust leadership team and can expect significant challenges in a changing Aged Care environment which needs to respond to a growing patient population and Commonwealth and State Health reforms impacting on operations.
