The Team: Care@home is a state-wide interdisciplinary service that remotely supports patients, to better self-manage their illnesses and chronic conditions in their own home using virtual care technologies, improving patient health outcomes, and reducing potentially preventable hospital admissions. This virtual care service has been established as a state-wide navigation hub, that provides a single point of access for home and community-based care; making it easier for both consumers and clinicians to identify what care pathways are available to support people in their preferred environment. You will need Current knowledge of, and the ability to apply nursing principles, procedures and practices in the delivery of patient/client care in a designated practice area and in line with legal requirements and the Australian Nursing and Midwifery Board (NMBA) Registered Nurse standards for practice. Sound interpersonal and communication skills, including written skills and the ability to function effectively in a multidisciplinary team environment. Knowledge of continuous quality improvement (safety and quality) and the application of evidence based practice in the practice setting.
