Aged Care Assessment is a multidisciplinary team that assesses frail clients aged for Indigenous clients) to determine their care needs for residential, respite, community care, and rehabilitation programs, aiming to facilitate access to appropriate services through comprehensive, client-centered evaluations involving clients, carers, and providers across Central Queensland.
About the role:
The Aged Care Assessment (ACA) Senior provides leadership, training, and support to clinicians, conducting assessments in hospital or community settings per Commonwealth guidelines and the Aged Care Act 1997, with a focus on social work principles, comprehensive care planning, and ensuring quality, evidence-based practice through effective communication and collaboration within the public health system.
About you:
Nurse Grade 7 - The successful applicant must hold a Nursing qualification and maintain the registration requirements with the (AHPRA) Australian Health Practitioner Regulation Agency, and Registration Board of Queensland including any necessary endorsements.
HP4 - The successful applicant must hold a tertiary qualification and maintain the registration requirements with the (AHPRA) Australian Health Practitioner Regulation Agency.
Previous Aged Care Assessment experience would be an advantage.
What we offer:
Total remuneration value of up to $164,212 p.a. is comprised of:
An hourly rate between $ $72.58 p.h.
