About the Role
This role involves providing expert clinical care to terminally ill patients and their families, ensuring their physical, psychological, and spiritual needs are met. The successful candidate will work closely with existing services involved in the patient's care, including Residential Aged Care Facility staff, supported residential service staff, and General Practitioners.
The Palliative Care Clinical Nurse Consultant will provide thorough assessments, identify current state and clinical care needs, implement interventions, and review progress in line with patient goals and wishes. They will also work collaboratively with other Alfred Health clinicians and teams to ensure seamless and timely care.
The ideal candidate will have a postgraduate qualification in Palliative Care Nursing and be registered as a Nurse Division 1 with a current practicing certificate. They will possess excellent written and verbal communication skills, and have the ability to work positively and flexibly with internal and external services.
Responsibilities
* Provide expert direct patient care, including thorough assessment, identification of current state and clinical care needs, implementation, administration, and review of interventions in keeping with patients' goals and wishes.
* Work closely with MATS Medical staff and patients' GP and RACF staff to ensure all clinical care needs are met as per the patients' preferences and teams' recommendations.
* Work collaboratively with other Alfred Health clinicians/teams, particularly our PCCS/PATS colleagues, to ensure seamless and timely care.
Requirements
* Registered Nurse Division 1 with a current practicing certificate with the Nursing and Midwifery Board of Australia via AHPRA.
* Postgraduate qualification in Palliative Care Nursing.
* Current Victorian driver's licence.
* Excellent written and verbal communication skills.
