Position Title: Physiotherapist (Aged Care – Home Care)
Reports to: Clinical Manager / Allied Health Lead

Position Summary
The Physiotherapist provides evidence-based, person-centred physiotherapy services to older people living in their own homes. The role focuses on maintaining mobility, improving function, preventing falls, managing pain, and supporting clients to remain independent and safely age in place, in line with Aged Care Quality Standards and relevant professional guidelines.
Key Responsibilities Clinical Assessment & Treatment
* Conduct comprehensive physiotherapy assessments in clients' homes
* Develop, implement, and review individualised treatment and reablement plans
* Deliver interventions including strength, balance, mobility, and pain management programs
* Provide falls prevention assessments and recommendations
* Prescribe and progress home exercise programs
Aged Care & Home Care Support
* Support clients with chronic disease management, post-hospital rehabilitation, and mobility decline
* Assess functional capacity and recommend mobility aids and equipment
* Provide education to clients, families, and carers on safe movement and injury prevention
* Support ageing in place and reablement principles
Collaboration & Care Planning
* Work collaboratively within a multidisciplinary team including RNs, ENs, OTs, care workers, and GPs
* Participate in case conferences, care reviews, and goal setting
* Provide clinical guidance to support workers within scope
Documentation & Compliance
* Maintain accurate and timely clinical documentation and reports
* Ensure services align with Aged Care Quality Standards, Charter of Aged Care Rights, and funding guidelines
* Comply with AHPRA registration requirements and APA Code of Conduct
* Adhere to infection control, WHS, and manual handling standards in home environments
Essential Requirements
* Bachelor or Master of Physiotherapy
* Current AHPRA registration as a Physiotherapist
* Current National Police Check
* Current First Aid & CPR certification
* Valid driver's licence and reliable vehicle
Desirable Skills & Experience
* Experience in aged care, community health, or home-based rehabilitation
* Knowledge of Home Care Packages, CHSP, and Support at Home Program
* Experience in falls prevention and reablement-focused care
* Strong time management and independent working skills
