Fixed Term Part Time – 12‐76 hours per fortnight (Available through 30th June 2027)
We are looking for a Health Care Worker to join our supportive Community Aged Care Services team at Portland District Health. As a Healthcare Worker – Community Support, you will assist consumers with:
* Light, general housekeeping such as sweeping, vacuuming, mopping, dusting, wiping, bed making, laundry, and limited shopping.
* Provide in home respite or participate in social activities like shopping, outings and walks that assist them to stay active and connected with their community: including one on one and/or group activities.
* Encouraging consumers to maintain and enhance their abilities and strengths.
* Encouraging consumers to actively participate where possible to maintain their independence.
* Support the individual's choice to remain at home and participate in their community.
* The incumbent shall have no authority to carry out personal tasks which would require the skills of a qualified nurse or allied health professional, nor to carry out any tasks for which he/she does not possess the necessary skills or level of competence.
Qualifications and Experience
* Certificate III or Certificate IV in Aged Care, from an Australian registered training organisation, or recognised equivalent qualification.
* Current Victorian Drivers licence.
* Demonstrated ability to practice collaboratively and work effectively as part of multidisciplinary health care team.
* Understands and embodies person centred care in practice skills that align registration requirements & PDH frameworks, policy, and procedures.
* Demonstrated understanding of the needs, issues, and sensitivities of people from diverse backgrounds and cultures.
* Excellent communication skills and ability to focus on delivering friendly, courteous, and respectful service to patients and residents.
* Previous experience working in hospital, community care, or residential aged care setting.

* Right to work in Australia
* Current Immunisation status. This role must meet the requirements of the Portland District Health Immunisation Policy.
