Advanced Pharmacist - Onc & Haem Clinics support & prescriber

To operate as an Advanced Pharmacist to deliver a highly specialist Pharmacy Service to Adult Oncology being an active member of the multidisciplinary team and thereby promoting the safe, appropriate and clinically effective use of medicines.

To lead the building of new and review electronic SACT prescriptions.

To organise, provide and evaluate the Independent Pharmacist Prescribing Service to the Oncology and Haematology Division; outpatients and day case chemotherapy.

To develop and evaluate the delivery of a responsive, proactive, patient-focused Independent Pharmacist Prescribing Service to support the adult oncology and haematology for patients receiving treatment for cancer in the outpatient and day case unit.

To be an active member of the multidisciplinary team promoting the safe, appropriate and clinically effective use of medicines.

To communicate highly complex information relating to medicines with clinicians, nurses, independent prescribers, carers and patients.

To support, develop and evaluate the delivery of a responsive, proactive, patient-focused clinical pharmacy service to the Oncology and Haematology directorate with the Principal Pharmacist.

To contribute to the effective medicines management by maintaining and developing key guidelines, protocols and PGDs.

Main duties of the job

Specialist Clinical Pharmacy Service to Adult Clinical Oncology To deliver a high level Specialist Clinical Pharmacist Service to Adult Clinical Oncology, including the provision of a daily Clinical Pharmacist Service to inpatients on the ward, outpatient clinics and day case chemotherapy.

To use specialist knowledge and expertise to make complex judgements and provide expert clinical advice, utilising highly complex information, evaluate evidence and to advise in situations when evidence is lacking in the specialist area of Oncology.

To lead the introduction of new SACT medicines and take the lead to develop new and review existing ensure Adult, and Oncology Clinical Trials treatment protocols to ensure they are safe and aligned with national, local Cancer Network protocols and NICE/CDF.

To prepare new or off-protocol prescription documentation, as required.

To monitor and report on off-protocol prescribing.

To adhere to local and national guidance and policies.

To take the lead for and work as a pharmacist independent prescriber in clinics and optimise the use of pharmacist independent prescribers in Oncology and Haematology clinics.

Job responsibilities

To provide pharmaceutical support and information necessary to manage medicines safely and cost-effectively.

Lead the development and implementation of medicine-related guidelines, policies, and procedures, particularly Pharmacist Independent Prescribing service.

Plan appropriate audit and practice research and implement appropriate service changes to Adult Oncology, specifically for Pharmacist Independent Prescribing service.

Set/monitor the standards for Pharmacist Independent Prescribing and use KPIs to show realisation of the Pharmacist Independent Prescribing Service model.

To make and communicate appropriate clinical interventions as part of the service provided and discuss with Pharmacists in the Cancer Services Team.

To support the Principal Pharmacist to investigate incidents and complaints relating to medicines usage within the specialist area and discuss with Pharmacists in the Cancer Services Team.

To implement changes in the current working practices within Pharmacy and chemotherapyprescribing, and encourage new ways of working, particularly Pharmacist Independent Prescribing service.

To supervise, check and, if necessary, undertake the supply of medicines.

To optimise medicines through prescribing role operating within areas of competence (where appropriately qualified and authorised via the trust NMP register).

Maintain an up-to-date clinical expert knowledge in Clinical Pharmacy and pharmacy practice to ensure a broad base of pharmaceutical knowledge and an advanced level of clinical reasoning and judgement.

To record your clinical activity and to monitor the outcome of your advice.

To fulfil the requirements of the Code of Ethics and Standards of Good Professional Practice.

To implement the Duty of Candour.

Medicines ManagementTo ensure all aspects of medicines usage within Oncology meet agreed National or local Trust standards.

To ensure that the principles of safe and secure handling of medicines and controlled stationery eg, prescription pads/CD books are adopted and to take part in the quality assurance systems for pharmaceutical services to comply with legislation and quality standards eg, Care Quality Commission.

To support the Principal Pharmacist to identify and develop opportunities where Oncology pharmacy staff can improve the flow and quality of patient care.To demonstrate awareness and commitment to the Trusts Clinical Governance agenda.

To participate in the analysis, identification and management of medication risks.

To manage the introduction of new medicines by undertaking formulary submissions, evaluating the clinical and financial impact in Secondary and Primary Care and identifying risks associated with new medicines.

To ensure that clinical and non-clinical incidents are reported.

To contribute to and support the development of business cases for new medicines and services.

Education, Training and Supervision

To act as a role model, supervisor and mentor for the Specialist Cancer Services (Oncology) Pharmacists, Rotational Pharmacists, Trainee Pharmacists, Medicines Management Technicians and other members of Pharmacy staff.

To support induction of staff into the Pharmacy Service for Oncology, and to identify training and development needs, to ensure staff are able to perform their duties and take responsibility effectively

Person Specification

Education, Training and Qualifications

* Master's Degree, MPharm or equivalent
* Registered with General Pharmaceutical Council.
* Diploma/MSc in Clinical Pharmacy or equivalent
* Independent prescriber currently practicing in Oncology/Haematology
* Mandatory CPD Portfolio
* Working towards RPS faculty recognition
* Member of British Oncology Pharmacy Association

Knowledge and Experience

* Extensive operational experience at a Senior Hospital Pharmacist level, demonstrating a high degree of autonomy
* Knowledge and experience of the pharmaceutical care of oncology patients
* Experience of supporting Divisional Pharmacist or equivalent, formulary application, guideline writing
* Experience of practice research or audits
* Good core clinical knowledge of therapeutics and pharmacology
* NHS arrangements, eg CCG, CQUINS, NICE, Clinical Governance
* Experience of prescribing within a specialty to optimise medicines for specific patient groups
* Electronic prescribing systems for chemotherapy
* Experience of electronic prescribing and administration systems and patient records
* Change Management
