About you:
In your current role you will:
Deliver evidence-based interventions and treatments, ensuring the highest standards of care and safety for patients with chronic conditions.
Facilitate and coordinate care among multidisciplinary teams, ensuring seamless integration of services and effective patient management.
Provide education and support to patients, empowering them with knowledge and skills for effective self-management of chronic diseases.
Adhere to clinical governance frameworks, participate in quality improvement initiatives, and ensure compliance with regulatory standards.
Contribute to the strategic and operational planning for chronic disease services, including resource allocation and management.
