About your opportunity
This is a casual Comprehensive Assessor position within the THHS Single Assessment Organisation.
The Organisation is a multi-disciplinary team providing assessment services to older Queenslanders in both community and hospital settings.
The assessment system aims to simplify and improve access to aged care services, while the framework is designed to streamline the process and enhance the overall experience for older Queenslanders.
In this role, you will:
Provide clinical and non-clinical home-based, face-to-face assessments of clients referred via the My Aged Care Gateway using the Integrated Assessment Tool (IAT).
Work with clients and their representatives under the Single Aged Care Assessment System to establish goals tailored to individual needs and develop appropriate strategies and actions, including referrals to aged care and non-aged care services.
Deliver advisory clinical support, education, and information to staff, service providers, stakeholders, clients, and carers.
