Job Description
An exciting opportunity exists for a dedicated and passionate professional to join our team as a Navigator in the Pathway to Good Health program.
The Navigator will play a critical role in ensuring that children and young people receive timely, culturally appropriate health assessments, referrals, and wrap-around care.
Working closely with health practitioners, the Navigator will support the implementation of quality, culturally-sensitive care plans for children and young people, advocating for families and empowering them to navigate complex health systems with confidence and trust.
The role will require strong communication and partnership-building skills, as well as the ability to work autonomously and take initiative in a collaborative team environment.
Key Responsibilities:
* Develop and implement culturally-sensitive care plans for children and young people
* Build relationships with stakeholders, including families, carers, and service providers
* Advocate for families and empower them to navigate complex health systems
* Contribute to a collaborative, multidisciplinary team to achieve program goals
Required Skills and Qualifications
To be successful in this role, you will possess:
* A Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care Practice or willingness to obtain
* Strong understanding of child protection or out-of-home care systems and associated health needs
* Proven ability to build trust, advocate for families, and coordinate care plans
* Excellent communication and partnership-building skills
* Ability to work autonomously and take initiative in a collaborative team environment
* Current Working With Children Check and Driver's Licence
* Willingness to obtain a satisfactory Police Check
