About Us


We're proud to be part of the Family Support Network (FSN), delivering integrated, coordinated secondary family support services to improve outcomes for families facing complex challenges. FSNs are a partnership between community sector services and the WA Department of Communities, providing earlier and more targeted support to families, with a particular priority for Aboriginal families and young people leaving care.


About the Role


As a Family Support Facilitator – Intensive Case Management (ICM), you will play a vital role in supporting families referred directly by the Department of Communities who require intensive, persistent, and practical support to keep their children safely at home.

This is a dynamic, hands-on role that provides in-home assessment, case planning, and intensive case management, working closely with families and key stakeholders including Aboriginal Community-Controlled Organisations (ACCOs), departmental staff, and other service providers.


Key Responsibilities

* Provide trauma-informed, culturally-safe, strengths-based in-home case management services.
* Complete comprehensive assessments and collaboratively develop family case plans.
* Lead case conferences and interagency meetings to coordinate support services.
* Deliver practical, persistent, and relationship-based support for families in complex circumstances.
* Build and maintain strong, respectful working relationships with Aboriginal families, ACCOs, and other stakeholders.
* Assist families in strengthening cultural connections and kinship networks.
* Maintain accurate and timely case records and documentation.
* Uphold child safety and wellbeing in all aspects of your work.


About You


You're a compassionate, resilient, and collaborative professional with a genuine commitment to the safety and wellbeing of children and families. You understand the complexities faced by families experiencing multiple challenges and have the skills to engage families in crisis.

Essential Criteria:

* Tertiary qualification in Social Work, Psychology, Counselling, or a related field.
* Demonstrated experience providing intensive, individualised case management to vulnerable families, particularly Aboriginal families.
* Strong understanding of Aboriginal culture, family dynamics, and the importance of culturally safe practice.
* Experience working with families affected by issues such as trauma, mental health, family violence, homelessness, or child protection involvement.
* Ability to manage challenging behaviours and emotional distress with empathy and professionalism.
* Strong communication, stakeholder engagement, and problem-solving skills.
* High level of computer literacy and case management documentation skills.
* Current Working with Children Check, Unrestricted Driver's Licence, and ability to travel as required.
