We have a proud 40 year track record operating in a suite of programs in disability, mental health, homelessness, social and affordable housing, family & domestic violence, child family and community services.

**Description**:
ABOUT THE ROLE

In this position you will build support coordination opportunities for NDIS participants in the Tweed area. Coordination Consultants work in a way that is reflective, and grounded in our organisational values. Through building relationships, following best practice approaches you will work towards providing the best outcomes for our clients.

**Skills and Experiences**:
**WHAT WE ARE LOOKING FOR**:
You will have an understanding of the NDIS and the ability to coordinate, manage and/or facilitate a range of supports to meet identified needs in accordance with participant plans. You'll be a great communicator, have a willingness to learn new skills and enjoy working as part of a cohesive team of experienced consultants. You will receive training and mentoring to develop and grow in your role.

**Criteria**:

- Certificate IV Disability / Mental Health / Community Services and or specialised skills to perform at this level.

Knowledge of using positive behaviour support to work with individuals with challenging behaviours.
Ability to understand the Equal Employment Opportunity and Work Health and Safety requirements relevant to the position.
Knowledge of Disability Standards.
Understanding of planning and organising own work to achieve work outcomes with the ability to work autonomously and as part of a multidisciplinary team.
Demonstrated interpersonal, active listening, conflict resolution, written and verbal communication skills (e.g. phone manner, file notes) with a focus on key stakeholder relationships.
An understanding of the NDIS and other relevant legislation.
Ability to research for programs and other service provider services.
Knowledge of Momentum Collective programs and services
Understanding of a person centred and human rights based approaches.
