Clinical Care Coordinator: Registered Nurse (Grade 5)

Full Time, Monday - Friday pm)

Yallambee has an exciting leadership opportunity for an experienced Registered Nurse in a Clinical Care Coordinator role. Our current Clinical Services team boasting an Executive Clinical Operations Manager, Clinical Care Coordinator and a Quality Manager (all the clinical support that you need is already there).

The successful applicant will be part of a dynamic clinical team of dedicated and caring staff who understand each resident's needs, goals and preferences to optimise their health and well being.

Yallambee Traralgon Village for the Aged Inc. is a community based Not-For-Profit service comprising of Margery Cole, residential aged care facility, and Independent Living Village. Yallambee is operated under the guidance of a voluntary Board of Directors and the Yallambee Board is committed to the provision of quality care, a home-like environment, the protection and promotion of residents' rights and to continuous improvement.

Key Performance Areas include:

* Working closely with the Executive Clinical Operations Manager to critically analyse systems, processes and practices of nursing and care staff to evaluate effectiveness in the delivery of optimal Resident Care
* Ensure compliance with the Strengthened Aged Care Standards, Yallambee's Policies and Procedures, Registered Nurse Standards of Practice and Yallambee's Vision, Mission and Values Policy.
* Manage incidents and complaints achieving optimal outcomes for residents, staff and drives continuous quality improvement.
* Proactive collaboration with Department Heads to continual improve resident experience and overall wellbeing
* Highest standard of documentation including admissions, assessments and care planning and AN-ACC related documentation
* Clinical expertise to coach, mentor and support clinical staff
* Ongoing Clinical Assessment, review and monitoring
* Ability to identify, manage and resolve problems.
* Liaison with other health professionals, residents and families
* Participation in auditing and continuous quality improvement activities
* Contribute to Roster Management, interviews and performance Appraisals
* Emergency Coordinator responsibilities
* Ability to mentor and support care staff
* Excellent standard of clinical management
* Ability to identify, manage and resolve problems

Key Selection Criteria:

Essential

1. Registered Nurses with current AHPRA registration with the Nursing and Midwifery Board of Australia.
2. Excellent acute clinical skills, including care of a deteriorating patient/resident, and end of life care.
3. Clinical expertise in the management of common medical conditions
4. High level computer literacy in Microsoft Office and Excel
5. Ability to mentor and develop others, foster teamwork and effectively work as a team member whilst providing leadership
6. Excellent verbal and written communication skills
7. Ability to identify, resolve and manage problems and resolve conflict situations effectively
8. Demonstrated organisation skills
9. Understanding of a person-centred, philosophy
10. Great time-management and adaptability
11. Excellent interpersonal and communication skills
12. Excellent leadership/management skills
13. Must have a person-centred approach to care and service delivery

Desirable:

1. Experience in aged care, preferably in a residential setting
2. Understanding / experience of AN-ACC
3. Understanding of quality & risk management systems and the Aged Care Standards Framework
4. Ability to facilitate meetings
5. Previous clinical experience in Aged Care
6. Previous leadership experience would be advantageous.
7. Knowledge and understanding of Acredia clinical documentation system would be advantageous.
