About the Role:
We are seeking a part-time (25 hours per week) Wellness Coordinator to cover a period of parental leave. The role is due to commence in early September for a maximum term of 12 months and will cover two retirement villages located in Prestons and Menangle. The responsibilities include:
* Providing effective assessment and planning to coordinate and support residents with individualized services to improve their independence, wellbeing, and achieve their goals.
* Identifying individual support needs, including goals, strengths, and vulnerabilities of residents.
* Developing support plans to coordinate access to services for residents.
* Creating an environment that fosters resident participation and well-being.
* Maintaining and providing regular updates to the Village Manager about residents' situations.
* Participating in leisure and recreational activities related to community inclusion as required by the Village Manager.
* Establishing and maintaining networks with community and local service organizations, volunteer associations, and other groups.
* Promoting and coordinating team workplace innovation, change, and continuous improvement activities.

About You:
To be considered, you will have:
* Experience in Retirement Living, Residential Care, Community Services, Home Care, or related disciplines.
* Demonstrated ability to facilitate effective, individualized support for positive resident outcomes.
* High-level negotiation skills and ability to liaise with stakeholders.
* Excellent verbal and written communication skills.
* Active listening, interpersonal skills, and patience with seniors.
* Valid Driver's Licence and access to a reliable vehicle.
