Central Goldfields Shire Council is looking to recruit a Coordinator Community Care for a month contract to coordinate the delivery of community care services to consumers within the municipality in accordance with council corporate plans, policies and relevant legislation, within Consumer Directed Care, Choice and Aged Care quality framework.
This role will see you assume responsibility for the Commonwealth Home Support programme, Home and Community Care Program for Younger Persons and Regional Assessment Services.
Reporting into the Manager Community Services, you will review Council's programmes and oversee the implementation of the Council's Positive Ageing Strategy and Disability Action Plan.
Key responsibilities include:
4. Leading and managing a team of competent community care staff implementing and maintaining policies and procedures relevant to the delivery of services and care within the community care department.
5. Coordinating the development of a Quality program
6. Taking responsibility for the financial management of CHSP and HACC PYP Program
7. Supporting community development initiatives
8. Developing functional business and service delivery plans
9. Administering grants and oversee funding acquittals/agreements
10. Coordinating projects and deliverables relevant to the Positive Ageing Strategy and Disability Action Plan
To be successful in this role you will hold a tertiary qualification in a discipline related to management, aged or disability services with strong experience in the planning, development and coordination of aged and disability programs and services. You will have solid knowledge of relevant government policy trends within the CHSP and HACC PYP services and programs, coupled with a passion for enriching lives and making a difference.
