We are seeking an Enrolled Nurse to join our medical centre in Caboolture.
About the Role
The successful candidate will be a key member of our clinical team, responsible for delivering high-quality patient care and managing chronic disease cases.
* Develop and implement care plans to help patients manage their conditions;
* Empower patients to take ownership of their health through coaching and education;
* Work collaboratively with GPs and other healthcare professionals to promote preventative healthcare;
* Conduct health assessments and identify early signs of chronic disease;
Key Responsibilities
1. Develop Care Plans: Prepare comprehensive care plans to support patients with chronic conditions;
2. Health Coaching: Coach patients on embedding healthy habits and behavior change principles;
3. Collaboration with GPs: Work closely with GPs to promote preventative healthcare and ensure seamless patient care;
4. Health Assessments: Conduct regular health assessments to identify early signs of chronic disease;
Requirements
* AHPRA registered Enrolled or Registered Nurse;
* Proven experience in Chronic Disease Management and health assessments;
* Strong organisational and time management skills;
* Excellent communication and customer service skills;
