Your opportunity – Be part of something bigger

Miwatj Health are pleased to offer this position as a permanent, full-time opportunity. Based in our Yirrkala Clinic, you will be working closely with our Aboriginal Health Workforce. To be successful in this position you will facilitate improved health outcomes for Aboriginal and Torres Strait Islander people with chronic health conditions. The care coordinator will provide clinical care, enable clients to gain better access to holistic, culturally appropriate multidisciplinary primary health care, health education and self-management strategies.

Core aspects of your role will include:

* Ensure enrolled clients have a current GP Management Plan.
* Using Management Plan goals support and empowers client to adhere to treatment regimes, develop chronic condition self-management skills and goals and connect with appropriate community-based services and allied health services.
* Arrange access to services, appointments and equipment as outlined in client’s care plans, consulting with client and their family or carer(s) as appropriate.
* Provide appropriate primary health clinical care and client participation in regular reviews by other primary healthcare providers.
* Record all patient interaction for reporting purposes and preparing a bi-annual report.
* Maintain accurate patient medical records in a timely manner.
* Provide feedback to client’s GP or other relevant health professionals.
* Case conferencing with relevant care providers and stakeholders.
* Support the addressing of social determinant factors impacting on client health.

Who we’re looking for:

To thrive in this role, you’ll have:

Essential minimum requirements

* Knowledge of health issues and health services, including the delivery of primary healthcare services for ATSI people.
* Highly developed interpersonal skills, including demonstrated liaison and negotiation skills in an environment requiring cultural sensitivity.
* Demonstrated effective communication skills within ATSI community and relevant primary health care providers.
* Strong data-gathering and analytical skills and the ability to contribute to reporting and evaluation of health programs and initiatives.
* Strong organizational skills with ability to set priorities and implement strategies to achieve best outcomes within specified time frame.
* Ability to work effectively both autonomously and collaboratively within team framework.
* Demonstrated ability to manage administration and budget requirements.
* Personal qualities including energy, initiative, genuine enthusiasm, respect for others, commitment to improving health outcomes for Aboriginal and Torres Strait Islander people.

Desirable

* Experience working in a remote Aboriginal community.
* Ability to speak an Aboriginal language from the region and/or knowledge of traditional kinship systems operating in the region.
* Demonstrated understanding of Aboriginal workforce training issues in an NT context.
* Demonstrated project management experience and organizational skills with ability to use initiative and to meet timelines with limited supervision.
* Demonstrated experience working with clients with chronic conditions.
* Previous experience in care coordination.
* Has undertaken cultural safety training.
* Experience delivering chronic condition self-management training and information.
