Remuneration: $116,639.97 - $120,469.88 per annum

Location: High Street Youth Health Service and Western Area Adolescent Team
Where you'll be working
Join the High Street Youth Health Service and Western Area Adolescent Team, where innovation and compassionate care meet to support young people and their families. You will be part of a multidisciplinary team delivering tailored health services within the Sustaining NSW Families (SNF) program.

What you'll be doing
In this senior clinical role, you will provide expert nursing care and leadership to support young parents and their children during pregnancy and early childhood, ensuring they are connected to the right care at the right time.
· Deliver advanced clinical care through sustained home visiting and outreach
· Build trusted partnerships with young parents to support health, development and wellbeing
· Provide clinical leadership, education and guidance to colleagues across Youth Health and SNF
· Conduct comprehensive developmental, health and psychosocial assessments
· Coordinate care and connect families with internal and external support services

Essential Criteria:
· Licence: NSW Class C driver's licence.
· Qualification: Postgraduate qualification in Child and Family Health Nursing
· Registration: Current enrolment with Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse Division 1
Selection Criteria:
1. Demonstrated capability and experience to engage with marginalised young people including young parents and their families where there are identified psychosocial risk factors and barriers, which impact on the families' engagement with mainstream services and the family's wellbeing.
2. A strong understanding of the developmental stages of adolescence, including physical, psychosocial, cognitive, social, and the barriers that may impact on young people and their family's engagement with services, parenting capacity and familial health issues.
3. Demonstrated ability to work effectively and collaboratively both independently and as part of the multidisciplinary health care team.
4. Demonstrated clinical expertise, and the ability to lead and foster innovation to enhance care provision within Youth Health and Sustaining NSW Families and the ability to provide complex care and advanced clinical skills in care planning and service provision for young parents and their infants/ children.
5. Demonstrated ability to provide leadership including education and quality improvement within the specialty and demonstrated effective communication including verbal, written and interpersonal skills and information technology skills.
6. Demonstrated experience in supporting learning and development through mentoring, training, or contributing to capacity-building initiatives within multidisciplinary teams.
7. Demonstrated proficiency in contemporary information systems, and a proven ability to design, implement, and evaluate specialised programs.
