Enrolled Nurse – Transition Care Program (TCP)
Brightwater Kingsley is a warm and welcoming home offering short-term care for individuals transitioning from hospital to home or to more suitable long-term accommodation.
We are currently seeking compassionate and experienced Enrolled Nurses to join our dedicated TCP team on a casual basis.
About the Role
As an Enrolled Nurse in our Transition Care Program, you will play a vital role in supporting clients during a critical phase of their recovery.
Your focus will be on promoting independence, restoring function, and helping clients achieve their personal goals.
TCP patients often have complex medical needs post-hospital discharge.
The environment is fast paced, acute style with high turnover inclusive of a broad range of clinical presentations.

The work is meaningful and rewarding, helping patients transition safely back to their homes or appropriate care settings.
We are currently offering part time work opportunities for successful and experienced candidates who are available to cover PM shifts, planned and unplanned leave.
The ideal candidate must have flexibility to work:
Week 1 Monday and Tuesday-3pm to *****pm
Week 2 Tuesday and Wednesday-3pm to *****pm
Key Responsibilities
Deliver person-centred nursing care under the supervision of a Registered Nurse
Support clients with daily living activities and rehabilitation goals
Monitor and report changes in health status
Exposure to rehabilitation, chronic disease management, and discharge planning enhances their versatility.
Autonomy and Decision-Making
Collaborate with a multidisciplinary team including physiotherapists, occupational therapists, and social workers
Assist in discharge planning and transition coordination
Maintain accurate documentation and care records
The successful candidate must have:
Current AHPRA registration as an Enrolled Nurse
Minimum 2 years' experience in aged care (TCP) or rehabilitation settings
Strong communication and teamwork skills
Empathy, patience, and a genuine commitment to client wellbeing
