Care Facilitator Role
This role is designed to empower clients by assisting them in engaging with services that enable them to remain living in their own homes.
The Care Facilitator will be responsible for developing individualized care plans and goals that focus on a well-living approach. This includes home visiting clients who have been referred for help at home, participating in service development and evaluation, and developing relationships with families and carers.
