Care Coordinator for Older Adults
The job is responsible for coordinating care for older adults in a variety of settings.
Key Responsibilities:
* Assist older adults and their families to navigate the healthcare system by providing information and resources about local healthcare services, such as hospitals, clinics, and home care agencies.
* Identify and address potential health risks and barriers to care through assessment and advocacy with healthcare providers, community agencies, and other stakeholders.
* Collaborate with healthcare teams to ensure continuity of care and promote safe transitions between healthcare settings.
* Engage with older adults, family members, caregivers, and healthcare providers to identify and prioritize needs and preferences related to care coordination, including advanced care planning and end-of-life care.
* Provide education and support to help older adults and their caregivers understand available care options, manage medications, and make informed decisions about their care.
* Support access to post-hospitalization aged care services, including residential aged care, sub-acute care, transitional aged care, and home care packages.
* Participate in quality improvement initiatives and contribute to evidence-based practice development to enhance care coordination services.
* Maintain accurate records and reports, ensuring data-driven decision-making and high-quality service delivery.
Requirements:
* Valid NSW driver's license.
* Applicable vaccination requirements for Category A roles
* Working With Children Check (WWCC)
* Current AHPRA registration
* Relevant post-registration qualification or at least three years' experience working in a relevant clinical area
