About the Role
As a Support Coordinator, you will work alongside participants, their families, and service providers to design and coordinate meaningful, goal-focused supports.
You'll help people understand and implement their NDIS plans and build their confidence, independence, and connection to community.
Key responsibilities include:
Building positive, respectful relationships with clients and stakeholders
Assisting individuals to navigate and implement their NDIS plans
Supporting clients to build capacity, strengthen informal supports and increase community connection
Helping clients make the most of their funding, including value for money and choice of providers
Managing documentation and communication via the NDIS Participant Portal
Maintaining strong, professional relationships with NDIA and relevant networks
About You
We're looking for someone with experience, empathy, and drive.
Ideally, you'll bring:
Confident communication, negotiation and advocacy skills
Ability to juggle competing priorities and manage your time well
Financial literacy and ability to help clients understand and manage their funding
Intermediate to advanced Microsoft Office skills and CRM systems
You must have (or be willing to obtain):
At least three years' experience working in the disability or community services sector
Support Coordination training is highly desirable, but we'll support the right candidate to complete it
A strong understanding of the NDIA and NDIS, including funding requirements, the practice standards, and pricing guidelines
NDIS Worker Screening Check
Blue Card (Working with Children Check)
Completion of the appropriate NDIS Worker Orientation Modules
Selection Criteria
Please respond to the selection criteria and give examples of your experience to demonstrate your skills and abilities, reflecting the duties associated with this role.
Extensive experience and demonstrated competency in working with people with disability and providing high quality services to meet their needs and goals
Demonstrated experience in working with participants and their families/ Nominees to identify their needs and assess risks and plan and coordinate services to meet their needs
Demonstrated understanding of the role of the NDIA and the NDIS, and how the NDIS practice standards are implemented to ensure compliance and positive outcomes for participants
Demonstrated ability to build rapport, effectively communicate and negotiate, and develop mutually beneficial relationships with participants, their families, Nominees, other service providers, and allied health professionals to meet the individual needs of participants
Strong critical thinking and problem solving skills, with experience in developing responses to addressing complex issues and complex support needs of people with disability
