**Clinical Care Partner Role Overview**
The Clinical Care Partner will provide relationship-centred clinical case management to a diverse client base, ensuring needs are met through comprehensive assessment, collaborative care planning, and support that promotes independence, choice, and quality of life.
Key Responsibilities:
* Delivering relationship-centred care aligned with Catholic Healthcare's Inspirit model
* Providing comprehensive support during client contact to resolve any additional outstanding matters, provide welfare checks, review of current services and monitoring for any change in circumstances
* Triaging and coordinating the provision of care management and services relating to short term pathways such as end of life and restorative care
Required Skills and Qualifications:
* Completion of tertiary qualifications in nursing or similar (Bachelor of Nursing) with a minimum of 3 years nursing experience
* Current Registered Nurse registration with AHPRA
* Thorough understanding of the Aged Care Legislation and Aged Care Quality Standards
