About the Role:
Providing evidence based mental health interventions, linking clients to primary health and psychosocial supports to meet the individual needs of those who experience severe mental illness. This position is community based and delivered by a team of experienced and credentialed mental health nurses and peer workers.
Key duties:
3. Accept referrals assigned by Intake and undertake due diligence on each referral.
4. Ensure effective delivery of quality, timely and culturally appropriate care to a diverse range of people who experience severe and enduring mental illness
5. Establish and maintain therapeutic relationships with proactive recovery oriented and trauma informed individual care, treatment and follow- up · Use professional risk assessment and best practice when engaging with clients and engage appropriate risk management follow-up
6. Include families, kinship groups and carers where appropriate and with consent, and provide support, education and/or referral to appropriate carer supports
7. Coordination of comprehensive holistic physical and mental health assessments, monitoring of mental and physical health and support to access any required treatments
8. Collaboratively engage with a range of support services including psychosocial as per each individual's assessed and expressed needs and refer as required
9. Work collaboratively and in partnership with LHDs, Peer Workers, GPs, Psychiatrists, Allied Health professionals, community agencies and other relevant stakeholders
10. Maintain clinical records in Salesforce and service contact data and outcome measures
Skills & Experience:
11. Credentialed with the Australian College of Mental Health Nurses (or evidence of working towards being credentialed within 12 months).
12. Minimum 5 years' experience as a mental health nurse within a community setting
13. Willing to complete Cultural Competency training & Trauma Informed Care in Practice training, if not done in last two years
14. Experience working with GPs and community-based psychiatrists
15. Ability to work in a team environment yet in a very autonomous role and self-manage a practice
16. Ability to work with and support peer workers
17. Ability to use internet based CiMS program effectively
18. Valid National Police Record Check, Working with Children Check, NDIS Workers Clearance, & appropriate COVID-19 Vaccinations
