Job Opportunity: Support Worker
Capital Region Community Services (CRCS) is a leading provider of high-quality programs and services to individuals, families, children and young people in the Capital Region.
As a Support Worker with CRCS, you will be responsible for assisting eligible older people to achieve their personal goals within a person-centred framework. This will involve developing trusting relationships with participants and service providers, providing information to facilitate access to community services and facilities, and working collaboratively with other team members.
Key Responsibilities:
* Assist participants in identifying, planning and implementing strategies to achieve their personal goals.
* Develop and maintain positive, professional relationships with participants and service providers.
* Provide information and support to participants and advocates as needed.
* Liaise with carers, families and significant others involved in providing care.
* Work collaboratively with other Support Workers within the team.
* Assist with problem-solving and addressing emergent issues.
* Identify communication or workflow issues and suggest improvements.
* Carry out administrative tasks including maintaining accurate case notes and files.
* Complete regular progress reports.
Required Skills and Qualifications:
* Hold or be willing to obtain a satisfactory Working with Vulnerable People registration.
* Undertake a National Criminal History Clearance by CRCS prior to commencing employment.
* Maintain full COVID-19 vaccination currency and provide evidence of vaccinations unless medically exempt.
