Job Summary:
We are seeking a dedicated Care Coordinator to join our team. The successful candidate will be responsible for coordinating, guiding and overseeing service provision to customers in need of care support.
The ideal candidate will have a strong knowledge of the aged care sector and experience in a coordination/case management role. They will possess excellent interpersonal and communication skills, with the ability to quickly learn and use new software for record keeping.
