About the roleWe are seeking a clinician to work in the Transitional Care Program (TCP).
This role involves both clinical work and client coordination.ResponsibilitiesRegularly liaise with Care Coordinators & external allied health / medical professionals.Share clinical knowledge.
Support, mentor & train Enrolled Nurses.Undertake wound care, monitor medication.
Assist with chronic illness & continence.Develop and maintain trust with clients.Go above & beyond to assist clients achieve desired health outcomes.About youProblem solving comes naturally, and you don't sweat the small stuff.
