Clinical Care Manager role. Full time position

Nurse Next Door Rockhampton is growing so we are on the hunt for a superstar RN, who will ensure that our clients are safe and happy in their own homes and lead a team of caregivers who love what they do. You may know similar roles as a Care Manager, Care Co-Ordinator or Clinical Care Manager. This role is a full time position. The role is ideally suited to an experienced RN/EN with fantastic leadership skills.

Your Next Adventure

Operating in a home care environment, you'll be responsible for ensuring that our clients are safe and happy in their own homes. You'll liaise with potential clients, set up individualised care plans to meet their unique needs and build professional relationships in the health industry.

Reporting to our Managing Directors, your role will be a combination of front line nursing work, management and development of our growing team of nurses and personal care assistants. This includes but will not be limited to, the following:

* Full time role

* Design tailored and meaningful client care plans that foster positive relationships between our clients and the Care Team.

* Meet with potential clients to consult on their options for premium home care services.
* Oversee client clinical care and maintain meticulous care documentation records.
* Clinical client visits
* Be accountable for maintaining the high clinical standards set by Nurse Next Door.
* Assist in the management and development of a growing team of caregivers and nurses.
* Believe in Making Lives Better for our clients every day, through thoughtful care and support.
* Celebrate Happier Ageing, enabling our clients to live a fulfilling life at home as long as possible, doing what they love to do.

Job Requirements

* AHPRA Registration
* Minimum 5 year post grad.
* CPR & First Aid Certification
* Current QLD Drivers License
* Residing in the Rockhampton/Yeppoon Region (We need you close by our clients)
* National Police Check (Aged Care/ Vulnerable Persons Sector)
* Working With Children Check
* NDIS Worker Orientation Module Completion + NDIS worker screen
* The nature of this role requires rotating on call evenings and weekends.
