Deliver Person-Centred Care
As a Clinical Care Partner, your primary focus will be to ensure that clients receive high-quality care by overseeing and coordinating complex clinical and care needs. This involves assessing, planning, and coordinating services to achieve the best possible outcomes for each client.
Your key responsibilities include:
* Developing and reviewing complex care plans in collaboration with clients and multidisciplinary care teams
* Managing budgets and facilitating multidisciplinary care activities
* Liaising with clinical teams to monitor and respond to changing client needs
* Continuously monitoring, reviewing, and evaluating care plans, services, and outcomes
* Assisting clients in navigating the care system and linking them to necessary resources
You will work closely with clients to understand their unique needs and develop personalized care plans that address these needs. Your role is critical in ensuring that our clients receive seamless and coordinated care.
Requirements
To be successful in this role, you must have:
* A Registered Nurse or Allied Health professional qualification with current AHPRA registration and unlimited work entitlements
* Demonstrated understanding and knowledge of client-centred, community-based care services delivery
* Demonstrated experience and knowledge in Community Funding streams and programs
* Excellent communication and clinical problem-solving skills
* A current Australian Driver's Licence and a roadworthy, insured vehicle for work purposes
Additionally, you should possess strong organisational and time management skills, enabling you to work autonomously and effectively manage competing priorities.
