The Role
Key Responsibilities:
1. Lead a Multidisciplinary Team: Provide leadership to a team of professionals across multiple locations, focusing on delivering high-quality care services.
2. Collaborate Across Teams: Work collaboratively with other teams to ensure a consistent approach to our care programs.
3. Develop a Culture of Empowerment: Foster a culture of empowerment, mentorship, and support for your team, prioritizing their wellbeing and safety.
4. Oversee Service Delivery: Ensure effective service delivery within the case management framework, reducing the impact of behaviours on individuals with dementia, their carers, and staff.
5. Contribute to Program Deliverables: Collaborate with other leaders to meet broader program needs and contribute to program deliverables.
6. Ensure Administrative Efficiency: Oversee the administrative aspects of the program, ensuring timely referrals, quality reporting, and financial goals.
7. Provide Direct Consultation: Offer direct consultation and support to clients as needed.
8. Enhance Personal Knowledge: Engage in ongoing learning and development to enhance your knowledge and skills in dementia care.
Requirements:
* Qualifications: A relevant health degree or qualification in nursing, allied health, or a related field.
* Experience: Experience in supporting older adults and carers with dementia, with a focus on psycho-social case management approaches.
* Leadership Skills: Proven leadership skills, with experience in leading and supporting teams across multiple locations.
* Communication Skills: Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
* Analytical Skills: Strong analytical and problem-solving skills, with the ability to manage complex situations.
* Customer Service: A commitment to providing excellent customer service, with a focus on client satisfaction and outcomes.
Desirable Requirements:
* Eligibility for Registration: Eligibility for registration with the relevant health practitioner registration body.
