Job Description
The Position
This role is responsible for assessing support needs, developing and implementing individual support plans for participants; consulting and liaising with the Clinical Care Coordinator in regard to all clinical matters; undertaking regular home visits, and liaising with participants and family members/carers regarding the implementation of support services provided; providing regular reports to the multi-disciplinary team and maintaining active communications regarding participants and service delivery; identifying and managing risks to participant safety; reviewing support needs of the participant twice yearly; ensuring compliance with relevant program standards and reporting; providing guidance and assistance to Support Workers (Aged and Disability); sourcing equipment and materials for participants, generating purchase orders and processing and reconciling invoices as required.
Required Skills and Qualifications
To be successful in this role you will need:
* Certificate IV in Aged Care or higher, or a relevant qualification.
* Demonstrated experience in coordination and supervision of employees, as well as an ability to train and mentor employees and work in a multi-disciplinary team.
* Demonstrated commitment to the health and wellbeing of the lives of older people and people with disabilities and their family members/carers.
* Strong communication skills with an ability to work with people from a diverse range of cultures and backgrounds.
* Demonstrated literacy, numeracy, accuracy and attention to detail.
* A high level of initiative, coupled with excellent interpersonal skills allowing the maintenance and development of good relationships with local stakeholders.
