• The Cardiac Rehabilitation Service, Chronic Conditions Service operates a Monday to Friday service, within the hours of 0630 to 1630. The Cardiac Rehabilitation Service is a multidisciplinary team providing expert, quality, accountable care and education in the Cardiac Rehabilitation management to patients of the Chronic Conditions Service across the care continuum and in accordance with prescribed professional and ethical standards and evidence-based practice

• Temporary Full-time chance for up to 2 months

About the role

The Clinical Nurse is a registered nurse who is accountable for practice at an advanced level, working collaboratively within the unit and with other members of the health care team including consumers/carers, other health professionals, general practitioners and other agencies (including non-Government agencies). This role provides the delivery of clinical care and support specific to Cardiac Rehabilitation within a Chronic Disease Management and Community Health setting.

About you

Our ideal candidate will be someone who can demonstrate the following:

- Demonstrated advanced clinical knowledge in the area of Cardiac Rehabilitation and Chronic Disease Management delivering expert nursing care in complex situations while providing support and direction to other team members.
- Demonstrated ability to role model excellent standards of comprehensive, evidence-based nursing care and documentation to facilitate quality patient outcomes, in accordance with National Safety and Quality Health Service and professional standards.
- Demonstrated ability to communicate effectively with members of the healthcare team, patients, relatives and visitors and maintain a positive workplace culture through well developed consultation, liaison, negotiation and conflict resolution skills.
- Demonstrated ability to support and lead quality improvement and research initiatives within the work unit, contributing to the use of evidence based practice to facilitate improved patient outcomes.
- Ability to prioritise clinical and related administrative duties, including effective and efficient planning and organisation of complex caseloads with proven ability to use information and reporting systems appropriate to the position.
- Demonstrates honesty, integrity and respect for all patients, carers and staff.
