Telehealth Services: Monitor and promote health outcomes through Telehealth consultations. Direct Care Services: Provide care across various funding programs, including wound care, diabetes management, medication management, stoma care, and catheter care. Client-Centered Care: Assess, develop, and evaluate nursing care plans collaboratively, focusing on client goals and independence with support from Allied Health clinicians. What you need to be considered for the role? Community Nursing Expertise: Strong knowledge of community nursing, primary health care, and consumer-directed care frameworks. Clinical Experience & Skills: Minimum 1 year's experience in community, acute, or aged care settings, with advanced clinical skills in chronic disease management, wound care, palliative care, stoma and catheter management, medication management, and diabetes management. Registered and Certified: Division One Registered Nurse with current AHPRA registration, valid driver's licence, current Police Check, and CPR certification. Independent yet Collaborative: Ability to work both autonomously and effectively within a multidisciplinary team environment. Tech-Savvy and Highly Organised: Proficient in using digital tools (smartphones, tablets, laptops) and client management systems, with a systematic, detail-oriented work style.
