An exciting opportunity in Child and Family Health to be part of a new multi-disciplinary team on the Central Coast, delivering sustained home visiting for children and families. This pilot project is an adaptation of the existing SNF program and works in partnership with parents to optimise the child's physical, social and emotional development and wellbeing.
The SNF Plus program commences during pregnancy until the child is 2 years of age. The program provides a more complex level service response to provide co-ordinated care for families. The Social Work role will work collaboratively with the Child and Family Health Nurse home visitor and wider multi-disciplinary team to deliver the structured program to families.
About the Opportunity
As a Senior Social Worker within the Sustaining NSW Families (SNF) Program, you will provide specialised assessment and intervention to support safe, healthy relationships and strong early childhood development. The role operates within an interdisciplinary team that delivers sustained, targeted home-visiting support from pregnancy until a child reaches two years of age. There are three temporary part-time positions available within the team.
In this role you will:
* Provide therapeutic assessment, counselling and intervention to strengthen parent-child attachment and overall family functioning.
* Deliver complex case coordination, individual and family therapy, group work and referrals to support holistic care.
* Apply advanced clinical judgement within a dynamic multidisciplinary environment to manage diverse and complex presentations.
* Contribute to service development, evaluation, and interdisciplinary education to enhance Social Work practice.
* Provide supervision, guidance and clinical leadership to junior Social Work staff and students.

* Proven ability to provide leadership, clinical supervision and support within a multidisciplinary service environment.
* Strong organisational, communication and documentation skills with the capacity to manage competing clinical demands.
