The Salvation Army's AOD & Mental Health Services Barwon plays a vital role in helping individuals regain stability, build independence and re-engage safely with their communities after periods of significant mental health crisis. By providing psychosocial support, case management and meaningful connection, this role directly contributes to reducing relapse, homelessness risk, and community isolation for people with complex mental health needs.**About the role**As a Mental Health Case Manager, this is your opportunity to provide psychosocial support and case management to clients transitioning from Barwon Health's acute Mental Health Unit and PARC services into program accommodation. You will also support TSA services across the Barwon region through mental health psychosocial secondary consultation, ensuring seamless service integration and client-centred practice.Reporting to the **Team Leader, Non-Residential Services**, this is a **Permanent, Full-Time**, onsite role located in Geelong, Victoria.**How you will make an impact**In this role, you will:* Conduct client assessments in partnership with Barwon Health to determine suitability for transitional housing programs* Provide case management including the development, implementation and review of individual case plans that support recovery and community reintegration* Support participants as they move from acute care into transitional accommodation and engage in structured program activities* Facilitate group activities and fortnightly house meetings to promote empowerment, strengths-based development and improved social inclusion* Maintain accurate case notes, SAMIS records, reports and contribute to program development, quality practice and compliance requirements**What you will bring*** A tertiary qualification in Welfare, Social Work, Psychology or a related discipline (desirable)* Demonstrated experience in mental health case management, preferably a minimum of two years in the mental health sector* Understanding of the homelessness sector and the intersection of accommodation instability with mental health recovery* Strong knowledge of family violence frameworks, including risk identification, assessment and management aligned with the MARAM framework* A current Working With Children Check and Victorian Driver Licence* Ability to work collaboratively within multi-disciplinary teams and build strong partnerships with external agencies**What we offer**The Salvos provide a well-rounded package of meaningful benefits, including:* Salary packaging up to $15,900 tax-free + $2,650 meal/entertainment benefit* Generous Paid Parental Leave (12 weeks primary, secondary carer leave)* Purchased Leave Scheme (up to 8 weeks) + 5 days paid leave annually to experience TSA programs* Access to EAP, health & wellbeing initiatives including Fitness Passport* Aboriginal & Torres Strait Islander supports including: Burra Burra Network, Cultural Events, Sorry Business & Ceremonial Leave (up to 10 days)* Ongoing professional development, supervision and training pathways* Rewarding and fulfilling purpose driven careers that have positive and sustainable / social impacts**How to apply**If you're passionate about making a difference and ready to help individuals rebuild their lives, we invite you to apply.
