About the role
We're looking for a dedicated Practice Nurse to join our Medical Services team in a 0.4 FTE fixed term position until 30 June 2026. This new role will support the delivery of chronic disease management (CDM), helping patients navigate long-term health concerns through high-quality care planning, health assessments, and follow-up support.
You'll work closely with our GPs, allied health staff, and the broader care team to coordinate and deliver compassionate, person-centred care, supporting prevention, education, and patient empowerment.
Hours: 15.2 hours per week (0.4 FTE)
Location: 23 Lennox Street, Richmond
Contract: Fixed Term until 30 June 2026
Classification: Community Health Nurse (ZJ1) – $55.98/hour
Closing Date: Thursday 31August 2025
About You
You're a Registered Nurse who is:
* AHPRA registered
* Experienced in community health or general practice (3+ years)
* Confident with CDM, care plans, and health assessments
* Skilled in immunisations, infection control and wound care
* Organised, collaborative, and passionate about working in diverse communities
