Total remuneration package of $99,965 - $108,781 per annum including superannuation and the full optional benefit of salary packaging.

We are looking for a self-motivated, experienced person who enjoys a diverse workload with lots of variety, working in the Support at Home program.
The responsibilities of a Care Partner are varied and each day brings new challenges. As a member of a larger team, you will oversee a portfolio of clients, ensuring that you provide the necessary support for them to safely stay at home while working towards their goals.
To be successful, you will require;
Appropriate qualifications compliant with Australian Qualifications Framework (e.g. Diploma of Case Management and/or Community Services/Nursing, or related field)
