A rewarding opportunity for a passionate Dementia professional to join HammondCare on a Permanent Full-time basis at our Miranda Site.
Dementia Consultant (Dementia Advisor)
Focusing on improving the lives of individuals living with dementia by applying dementia‐specific clinical knowledge and skills in a residential aged care setting. As a key member, you will mentor care staff to enhance their support and implement HammondCare's Philosophy of care and Case management framework. You will work closely with teams across the organisation, build capacity, share expert knowledge, and contribute to the development of evidence‐based care practices.
Responsibilities
* Provide dementia‐specific expert advisory services within HammondCare services, through participation in regular clinical review meetings.
* Coach staff, building their ability and capacity to deal with dementia related situations and escalations; build staff capacity through communication, education, mentoring and coaching in practice; this can include spending time with the team on the ground and lead by example with care support, and also conducting case study discussions and review of documentation.
* Plan engagement with individuals and teams to achieve agreed practice development objectives and quality improvements.
* Participate, contribute and learn as part of the Residential team in team meetings, professional supervision and team activities.
* Evaluate and report by utilising HammondCare processes to communicate, monitor and manage work delegations and projects including evaluation and reporting.
* Participate in projects and behaviour management planning.
Qualifications and Experience
* 3 years of proven experience working with people living with dementia is essential.
* Registered Nurse or Allied Health qualification essential; further qualification in Training and Assessment is highly desirable.
* Demonstrated skills in teaching, mentoring and coaching others.
* Confidence with conducting care tasks such as personal care and resident general support.
* High level of negotiation, communication and relationship building skills.
* Problem solving and decision‐making skills relevant to the aged and dementia care context.
* Strong interpersonal and communications skills both written and verbal.
* Strong administrative and computer skills using Microsoft Office products.
* Ability to work independently and as part of a team.
