About us
Located within Bolton Clarke Sunset Ridge Residential Aged Care Facility, the 30‐bed Interim Care Unit provides compassionate, around‐the‐clock support for patients who are ready to leave the acute hospital setting but still need ongoing care and guidance before their next step.
We support people who are unable to return home immediately and require assistance transitioning either into a Residential Aged Care Facility (RACF) or back into the community with National Disability Insurance Scheme (NDIS) supports. Our team works closely with patients and families to help make this transition as smooth, respectful and comfortable as possible.
Our unit is made up of two welcoming wings – Seahorse, a secure area, and Starfish, a non‐secure area – each focused on creating a safe, supportive and homelike environment.
About the role
* Design, implement and evaluate a variety of recreational, diversional and social activities to improve or maintain the physical and mental function of the patients of Sunset Ridge Interim Care unit.
* Ensure a record of all recreational equipment and supplies is maintained and all equipment is kept in a hygienic condition and good working order.
* Provide assistance and on‐the‐job training for volunteers, visitors and students on placement who assist with the activities programme.
* Liaise and network with community groups and organisations to maintain patient contact and involvement with the local community.
* Utilise feedback from patients and relative forums and satisfaction surveys to critically appraise recreation programmes.
About you
* While there are no mandatory qualifications for this role, it is highly desirable that the applicant possess, or be working towards, the qualifications listed in the role description.
* Assess individual patient's abilities, needs and interests and adapt activities accordingly.
* Demonstrate the ability to initiate, develop and implement lifestyle programmes for residents with chronic cognitive deficits.
* Bring a proven ability to work effectively as a member of the team and promote and adapt to change.
* Demonstrate knowledge of the process and strategies of continuous improvement of clinical services and demonstrate ongoing involvement in this process.
What we offer
* Total remuneration value of up to $82,130 p.a., comprised of an hourly rate between $34.53 – $36.30 p.h., 12.75% employer superannuation contribution and 17.5% leave loading.
