The Opportunity Are you a skilled Registered Nurse ready to take the next step in your career? Our client is seeking a passionate Case Coordinator to lead a clinical team in delivering integrated community care to clients with complex acute and aged care needs. In this pivotal role, you will coordinate clinical and physical resources, provide direct client care, and act as a key escalation point for clinical challenges. You'll work closely with the Clinical Nurse Manager to achieve service delivery goals and uphold our client's commitment to excellence. This role operates Monday to Friday, between 7am and 4pm. Key Responsibilities Lead and support a dedicated clinical team, fostering growth, competence, and emotional wellbeing Coordinate resources to deliver person-centred, evidence-based care that meets client needs Deliver hands-on clinical care and resolve complex issues with confidence and compassion Act as a key escalation point for nurses managing client deterioration and other clinical challenges Drive effective communication through team meetings, stakeholder engagement, and thorough documentation Support onboarding, mentoring, and student placements to build a strong, future-ready workforce About You You bring passion with purpose and are driven by excellence.
